Cardiology Intervention
Interventional cardiology is a branch of cardiology that deals specifically with the catheter based treatment of structural heart diseases. A large number of procedures can be performed on the heart by catheterization. This most commonly involves the insertion of a sheath into the femoral artery (but, in practice, any large peripheral artery or vein) and cannulating the heart under X-ray visualization (most commonly fluoroscopy). The radial artery may also be used for cannulation; this approach offers several advantages, including the accessibility of the artery in most patients, the easy control of bleeding even in anticoagulated patients, the enhancement of comfort because patients are capable of sitting up and walking immediately following the procedure, and the near absence of clinically significant sequelae in patients with a normal Allen test.
The main advantages of using the interventional cardiology or radiology approach are the avoidance of the scars and pain, and to plan early patient discharge. Additionally, interventional cardiology procedure of primary angioplasty is now the gold standard of care for an acute myocardial infarction. It involves the extraction of clots from occluded coronary arteries and deployment of stents and balloons through a small hole made in a major artery, which has given it the name "pin-hole surgery" (as opposed to "key-hole surgery").
Interventional procedures
A microtube is inserted along the arteries in the hand or thigh, placed at the origin of coronary arteries and contrast injection is given to visualise the blockade in the coronary arteries.It is the gold standard for diagnosing the Coronary Artery Disease.
The miniaturised balloon is threaded along the wire placed in the coronary Arteries and chlosterol plaque is redistributed by inflating the balloon, followed which Medicated stents(Drug eluting stents) are deployed to prevent recoiling of treated segments.
In order to treat symptomatic slow and irregular heart rhythm, an artificial pulse generator is implanted in the anterior chest wall,subcutaneously and leads are positioned in the right atrium and Ventricle and heart rhythm is stabilised. Life of the pacemaker may vary from 7 – 15 years depends upon the need for pacing and pulse Generators can be replaced simply.
Using the minimally invasive technique,small button like device can be positioned across the defect using special delivery cable and defects are sealed off successfully.
A light source (Near infra red spectrum) is positioned distally in the coronary arteries and mechanised pull back is done. Reflected light from the vessel wall is transformed into characterisation of layers of arteries and mechanism of occlusion of arteries-Lipid, fibrous tissue, calcium or thrombus. And to ensure adequate deployement of stents.
Ultrasonic waves is used to acquire images of vessels inside the artery. IVUS provides detailed and accurate measurements of lumen and vessel size, plaque area and volume, and location of key anatomical landmarks. This technique is very useful to study vessel wall calcium and plaque burden. This assesment is essential for performing complex Angioplasties.
The radiofrequency energy is delivered to eliminate abnormal electrical activities within the heart. This act by necrosing the abnormal tissue and minimal scar formation. Very efficient method and ensures complete cure in 99 % of patients.
Very similar to pacemaker, but delivers electrical energy to terminate chaotic ventricular activity (VT/VF) causing cardiac arrest. This is the life saving procedure and definitely indicated in the patient surviving with very low Ejection fraction (Low Cardiac Pumping Function).
